Doctor Whooves and the Crazy Fan

Whenever me and my sister watch My Little Pony, i am always on a Doctor Whooves watch, whether he has a speaking part, or he's a teeny tiny background character in a scene.
However, she'll inform me that most times, it's just a fan, and that Doctor Whooves wears a tie...
So, i figured that this can also mean that Doctor Whooves and a fan have a good chance of seeing each other. Does Doctor Whooves exist as a show in their world though? Or are they stories in tales of the 'oncoming storm'? I know, at least, that if i were a fan in this world, i would most likely react like this to Doctor Whooves!
